Applicants can apply as an Individual and/or as part of a Multidisciplinary Team. If your Team is selected, all members of the Team will be awarded a scholarship. If you desire a Team Scholarship, only one person needs to complete an application that includes information for each Team member. Only one scholarship can be offered per applicant, regardless of how many ways you apply.
Applications will be accepted through November 10, 2017.
Applicants and the MDT Point of Contact (POC) will be notified by November 15, 2017 via email whether or not they have received a scholarship.
